Domiciliary Care at Home

Specialist support tailored to your routine, helping you live independently and confidently at home.

  • Flexible support tailored to individual needs
  • Specialist care for complex conditions and disabilities
  • Helping people live safely and independently at home

What Is Domiciliary Care?

Domiciliary care, sometimes known as home care, provides support within a person's own home at agreed times throughout the day or week.

Support can range from help with everyday activities through to more specialist care for individuals living with complex health conditions, injuries or disabilities.

Our domiciliary care services provide flexible, personalised support for adults, children and young people with a wide range of complex needs, delivered in the comfort and familiarity of their own home.

Care That Fits Around Your Life

Every person's circumstances are different, which is why no two packages of care look the same.

Support can be tailored around:

  • Morning and evening routines
  • Education or employment
  • Family commitments
  • Rehabilitation programmes
  • Healthcare appointments
  • Community activities
  • Personal goals and interests

Rather than expecting people to fit around a service, we build support around the individual's life, preferences and ambitions.

Supporting Complex Needs at Home

Many people are living with multiple conditions and require a coordinated approach involving family members, healthcare professionals and wider support networks.

Our focus is on understanding the whole person and delivering support that reflects their individual circumstances and goals.

Many of the individuals we support live with complex needs arising from:

  • Brain Injury Care

Helping individuals rebuild confidence, maintain independence and continue rehabilitation following acquired or traumatic brain injury.

  • Mental Health Support

Supporting people with mental health conditions to live independently, maintain stability, and rebuild confidence in everyday life.

  • Neurological Care

Specialist support for people living with neurological and neurodegenerative conditions, helping them maintain independence, wellbeing and quality of life at home.

  • Physical Disability Support

Specialist support for people with disabilities, that promotes independence, opportunity and quality of life.

  • Spinal Cord Injury Care

Supporting recovery, independence, and quality of life through specialist, clinically-led care.

Promoting Independence and Quality of Life

Care should be about more than meeting immediate needs. Our approach focuses on supporting individuals to remain as active, independent and engaged as possible.

This may involve:

  • Maintaining daily routines
  • Accessing education or employment
  • Attending appointments
  • Building confidence
  • Developing skills
  • Participating in hobbies and interests
  • Staying connected to family, friends and the community

We believe that the right support can help people continue living fulfilling lives while receiving the assistance they need.

Clinical Oversight and Specialist Support

Active Assistance is dual-registered for both Personal Care (DC) and Treatment of Disease, Disorder or Injury (TDDI). This enables us to support individuals with more complex clinical needs than many traditional domiciliary care providers.

Our services are supported by:

  • Nurse-led clinical oversight
  • Clinical competencies and training
  • Care planning and risk management
  • Regular reviews by qualified healthcare professionals
  • Governance and quality assurance processes

This helps ensure care remains safe, effective and responsive to changing needs.

Our Approach to Domiciliary Care

  • Flexible Support Built Around the Individual

Every package is tailored around the individual's routines, goals and lifestyle, providing the right level of support at the right time.

  • Promoting Independence and Confidence

Our focus is on helping people remain as active, independent and engaged as possible while providing support where it is needed.

  • Supporting Complex Needs

As a provider registered for both Personal Care and Treatment of Disease, Disorder or Injury (TDDI), we support individuals whose needs extend beyond traditional domiciliary care.

  • Clinically-Led Oversight

Our nurse-led approach, specialist training and regular reviews help ensure care remains safe, effective and responsive to changing needs.

  • Multidisciplinary Working

We work closely with therapists, clinicians, case managers and families to ensure support remains coordinated and aligned with wider goals and outcomes.

  • Care That Evolves over Time

Needs can change due to rehabilitation, illness, injury or changing circumstances, so support is regularly reviewed and adapted to reflect the individual's current needs and aspirations.

Frequently Asked Questions

Domiciliary care involves visits at agreed times throughout the day or week, while live-in care involves a dedicated Support Worker living in the individual's home.

Many people find domiciliary care provides the right level of support while allowing them to maintain independence. If needs become more complex over time, support can be reviewed and adapted accordingly.

Yes. Many of the individuals we support have complex clinical, neurological or rehabilitation needs that go beyond traditional home care services.

Our dual registration for Personal Care and Treatment of Disease, Disorder or Injury (TDDI), together with nurse-led oversight and specialist training, enables us to support more complex packages of care in the home environment.

Absolutely. We regularly support individuals transitioning home following hospital treatment, specialist rehabilitation or significant changes in health.

The right support can help rebuild routines, maintain rehabilitation goals and provide reassurance during what is often a significant adjustment period.

We recognise that building trust and consistency is incredibly important. Where possible, we aim to provide continuity in the people delivering care, helping individuals build positive relationships with their support team and feel comfortable with the care they receive.

Consistency can be particularly important for people living with a neurological condition, autism, a brain injury, or a mental health condition, where familiarity and routine can have a significant impact on wellbeing.

We recruit specifically for each package wherever possible, taking into account the individual's needs, goals, personality and environment.

All Support Workers receive mandatory training, package-specific training and ongoing competency assessments to ensure they can deliver safe, effective and person-centred support.

Needs can change over time due to recovery, rehabilitation progress, changing health conditions or personal circumstances.

Care plans are reviewed regularly by qualified healthcare professionals and can be updated whenever there is a significant change in health, risk, goals or support requirements. This helps ensure support remains aligned with the individual's needs and desired outcomes.

As part of Active Care Group, we can also help individuals, families and professionals explore wider support options if circumstances change significantly, helping provide continuity and reassurance for the future.

Yes. Our services are regulated by the relevant care regulators, including the Care Quality Commission (CQC) in England and the Care Inspectorate Wales (CIW) in Wales.

This provides reassurance that care is delivered within robust clinical, governance and safeguarding frameworks, and is regularly assessed against recognised standards for quality, safety and effectiveness.

Active Assistance is registered for both Personal Care and the Treatment of Disease, Disorder or Injury, reflecting our ability to support individuals with complex health needs as well as day-to-day care requirements.

Your first conversation with us is simply an opportunity to discuss your circumstances and explore the support options available.

We'll take the time to understand your needs, answer your questions and explain the services that may be appropriate. If required, we can then arrange an assessment and develop a personalised package of care.

Funding arrangements vary depending on individual circumstances.

Domiciliary care may be funded privately, through NHS Continuing Healthcare, local authority funding, direct payments, case management arrangements or personal injury and insurance settlements.

Our team can help explain the different options available and guide you through the process.
