Living with Brain Injury

A brain injury can have a profound and lasting impact - not just on physical health, but on confidence, relationships and day-to-day life. Whether you are preparing for a return home following rehabilitation, or need ongoing support for complex needs, our experienced teams provide personalised home care designed to help you live as independently as possible.

Understanding Brain Injury and its Impact

A brain injury refers to damage to the brain that affects how a person thinks, feels and functions. It can occur suddenly due to trauma - such as a fall or accident - or as a result of medical conditions like stroke, infection, or lack of oxygen.

There are two main types of brain injury:

The impact can vary widely, but often includes a combination of physical, cognitive and emotional changes. Some individuals may experience reduced mobility or fatigue, while others may face challenges with memory, communication or behaviour.

These challenges can make everyday activities more complex and often require specialist support to manage safely at home.

  • Traumatic brain injury (TBI): caused by an external force, such as a road traffic accident or fall

  • Acquired brain injury (ABI): caused internally, for example through stroke, infection or illness

When Is Specialist Brain Injury Care Needed?

The need for support often becomes most apparent during key moments - particularly when returning home after hospital or rehabilitation. At this point, families are often adjusting to new routines, increased care needs, and the emotional impact of a life-changing condition.

As part of Active Care Group, we can support people stepping down from Active Neuro services, helping to ensure continuity between rehabilitation and life at home. For those continuing their recovery journey, we can also work alongside Active Neuro's outpatient therapy services to help maintain progress and support longer-term goals.

Specialist brain injury care can also help when:

  • care needs become more complex or difficult to manage
  • safety or independence is a concern
  • family members need additional support or respite

Getting the right support in place early can make a significant difference to both recovery and long-term quality of life.

Supporting Independence

Working with Case Managers and Healthcare Professionals

We work in partnership with case managers, clinicians and wider professional teams to deliver structured, high-quality care.

From initial assessment through to ongoing reviews, our focus is on clear communication, coordinated care planning, and achieving the best possible outcomes for every individual.

Frequently Asked Questions

Brain injury care at home provides specialist support for people living with the effects of an acquired or traumatic brain injury, helping them manage daily life while continuing their recovery in a familiar environment.

Support is tailored to the individual and may involve assistance with mobility, communication, cognition, behaviour, rehabilitation goals, or more complex clinical needs. Rather than taking a one-size-fits-all approach, care is designed around the person's lifestyle, aspirations, and level of independence.

Every situation is different, but we understand that support is often needed during periods of significant change, particularly following hospital discharge or rehabilitation.

Once we understand your circumstances and complete an assessment, we will work with you to put the right support in place as quickly as possible. Our team can also help guide families through the process, providing reassurance during what can often feel like an overwhelming time.

Yes. Many of the individuals we support have needs that extend beyond standard home care. This may involve coordinating multiple aspects of support, managing complex physical or cognitive challenges, or adapting care as circumstances change over time. Our role is to bring these elements together into a structured, personalised care plan that promotes both safety and independence.

Yes. In fact, some of the most meaningful rehabilitation happens in everyday life. Home provides opportunities to apply skills and strategies in real-world situations, whether that's building confidence with daily tasks, improving mobility around the home, or reconnecting with hobbies and activities that are important to the individual.

We work closely with wider clinical and therapy teams to ensure progress made during rehabilitation continues after returning home. Where appropriate, individuals may also continue to access Active Neuro's outpatient therapy services. Our teams can work alongside therapists and wider professionals to help integrate rehabilitation goals into day-to-day life at home.

In many cases, yes. With the right support in place, many individuals can continue living safely and successfully in their own homes while working towards increased independence and rehabilitation goals.

Every situation is different, but home-based rehabilitation support can often provide an effective alternative to more restrictive environments while helping people remain connected to their families, communities and support networks.

We regularly work alongside occupational therapists, physiotherapists, rehabilitation teams, case managers and other healthcare professionals to ensure support remains coordinated and focused on agreed outcomes.

This multidisciplinary approach helps create consistency, supports rehabilitation goals and ensures everyone involved is working towards the same objectives.

Families are often an important part of the individual's support network.

We work collaboratively with families, helping them feel informed and involved while also providing practical support and reassurance that care is being delivered safely and consistently.

Yes. Our services are regulated by the relevant care regulators, including the Care Quality Commission (CQC) in England and the Care Inspectorate Wales (CIW) in Wales.

This provides reassurance that care is delivered within robust clinical, governance and safeguarding frameworks, and is regularly assessed against recognised standards for quality, safety and effectiveness.

Active Assistance is registered for both Personal Care and the Treatment of Disease, Disorder or Injury, reflecting our ability to support individuals with complex health needs as well as day-to-day care requirements.

For many people with a brain injury, their care needs are likely to change over time. Our care plans are reviewed regularly and updated whenever there is a change in health, circumstances, rehabilitation goals or support needs, helping ensure care remains aligned with the individual's recovery journey.

Your first conversation doesn't need to be a commitment to care.

We will take time to understand your situation, answer any initial questions and discuss the support you may need. From there, we can arrange an assessment and recommend a care plan tailored to the individual.

For many families, that initial conversation provides valuable clarity and reassurance at a time when there can be a lot of uncertainty.

Funding arrangements vary depending on individual circumstances.

Care may be funded privately, through an insurance or personal injury settlement, or via NHS or local authority funding pathways. Our team can talk you through the options available and help you understand what may be appropriate in your situation.
