Specialist Home Care in Wales

Even in cities like Cardiff, Swansea and Newport, specialist home care can be hard to find. In rural areas such as Powys, Pembrokeshire and Gwynedd, families often worry about whether a provider can offer care that is both specialist and reliable.

At Active Assistance, we specialise in supporting individuals with complex conditions in their own homes. Our focus goes beyond delivering care safely. We help each person maintain independence, rebuild confidence and live as fully as possible in the home and community they know.

From Cardiff neighbourhoods such as Llandaff and Pontcanna to Penarth, Cowbridge, Abergavenny, Mumbles and Llandudno, we support people in a wide range of settings, from city homes to coastal properties and rural farmhouses.

Complex and Specialist Care at Home

Our expertise lies in supporting individuals with complex, long-term and high-dependency needs - delivering care that goes beyond standard home care services.

We regularly support people living with:

  • Brain and spinal cord injuries, including acquired and traumatic brain injury
  • Neurological and neurodegenerative conditions, such as Parkinson’s disease, multiple sclerosis and MND
  • Respiratory and ventilation needs, including tracheostomy and assisted ventilation
  • Complex physical disabilities requiring ongoing clinical input
  • Learning disabilities and associated health needs

We also support individuals in situations where care needs are evolving or more intensive, including:

  • Transitioning home following hospital discharge or rehabilitation
  • Continuing rehabilitation within the home environment
  • Requiring 24-hour or live-in care
  • Managing cognitive or behavioural changes

Each care package is designed to support both clinical and day-to-day needs, helping individuals live as independently and confidently as possible at home

Supporting Individuals Across Wales

With the right support in place, home can become a place of progress and stability.

We have supported individuals across Wales to rebuild their independence following life-changing conditions - from returning home after prolonged hospital stays to re-establishing everyday routines and reconnecting with their communities.

They have made such a difference to our lives – we are very grateful that she is so well supported.

Yes. Active Assistance specialise in supporting individuals whose needs go beyond standard home care, including those requiring ongoing clinical input, coordinated care packages, or specialist expertise.

This can include situations where care involves multiple elements - such as managing physical health needs alongside cognitive or behavioural changes, or supporting individuals whose needs are evolving following illness, injury or treatment.

Our role is to bring these elements together into a single, well-structured care plan - ensuring support is consistent, safe, and able to adapt over time. Where appropriate, we also work alongside other professionals involved in the individual’s care to maintain a joined-up approach.

Yes. Live-in care is available across Wales, including rural Powys, Pembrokeshire and North Wales, where it can be the most dependable way to secure continuous support. A dedicated carer lives in the home and provides around-the-clock support, with clinical oversight from our nursing team.

Yes. Our services in Wales are regulated by Care Inspectorate Wales (CIW), the independent regulator of social care and childcare in Wales.

CIW inspects services under the Regulation and Inspection of Social Care (Wales) Act 2016, checking that care is safe, effective and focused on the wellbeing of the people receiving it. Care workers delivering care at home in Wales are also registered with Social Care Wales, the workforce regulator.

For families, this offers an additional level of confidence, particularly when arranging care for complex or high-dependency needs.

Your first conversation doesn't need to be a commitment to care.

We will take time to understand your situation, answer any initial questions and discuss the support you may need. From there, we can arrange an assessment and recommend a care plan tailored to the individual.

For many families, that initial conversation provides valuable clarity and reassurance at a time when there can be a lot of uncertainty.

Funding arrangements vary depending on individual circumstances.

Care may be funded privately, through an insurance or personal injury settlement, or via NHS and local authority funding pathways in Wales. This can include funding from Local Health Boards, direct payments, joint funding arrangements or other forms of statutory support.

Our team can help you understand the options available, explain how the process works and discuss the most appropriate funding route for your situation.
