Living with a Mental Health Condition

Living with a mental health condition can affect many aspects of day-to-day life, from managing routines and relationships to accessing education, employment and community activities.

We provide personalised support for individuals living with complex mental health conditions, helping them maintain independence, develop confidence and continue living in their own homes and communities. This includes people who have conditions such as:

As a provider of complex care, many of the people we support have complex physical health conditions in addition to their mental health needs.

Supporting Transitions from Hospital to Home

Leaving an inpatient setting can be a significant milestone. While returning home is often a positive step, it can also bring uncertainty. Re-establishing routines, reconnecting with the community and adapting to day-to-day life can take time.

We provide practical support for individuals transitioning from inpatient mental health services back into their own homes, helping them build confidence and maintain stability during this important period.

Working alongside community mental health teams and wider professionals, our goal is to support successful transitions and help individuals move forward with confidence. This aligns with wider mental health discharge and community-based recovery pathways

No. We do not provide counselling, psychotherapy or psychiatric treatment directly.

However, we regularly support individuals who receive treatment from mental health professionals and can help them engage with wider services, attend appointments, and maintain the stability and routines that support their recovery.

Yes. Returning home after an inpatient admission can be a significant adjustment. We provide structured support to help individuals rebuild routines, regain confidence, and re-establish independence while living in the community. This type of step-down support is a recognised part of recovery-focused pathways.

For some individuals, yes. With the right level of support in place, many people can continue living in their own homes rather than moving into a more restrictive environment. Support is tailored to the individual's needs and can adapt over time as circumstances change.

Absolutely. Home-based support does not mean treatment or therapeutic input has to stop. Many of the individuals we support continue to receive care from community mental health teams, psychiatrists, psychologists, therapists and other healthcare professionals while living at home.

Our role is to provide the day-to-day support that helps people put treatment plans into practice, maintain routines, attend appointments and work towards their personal goals. Where appropriate, we work alongside wider professionals involved in the individual's care to ensure support is coordinated and aligned with their ongoing treatment and recovery.

Yes. Many of the people we support have mental health conditions alongside other needs, including autism, learning disabilities, acquired brain injuries, physical disabilities, and complex health conditions.

In these situations, support needs to take account of the whole person rather than focusing on a single diagnosis. Our teams work to understand how different needs affect daily life and create a coordinated support plan that reflects the individual's circumstances, goals, and preferences.

Mental health needs can change over time, which is why support should never be static. 

Care plans are reviewed regularly and can also be updated following changes in circumstances, wellbeing, personal goals or professional recommendations. This helps ensure support remains relevant, personalised and focused on positive outcomes 

While many people can live successfully at home with the right support in place, circumstances can change. As part of the wider Active Care Group, we can work with individuals, families and professionals to explore other options where appropriate, including supported living, residential services or specialist treatment pathways.

This provides continuity and reassurance that support can evolve alongside the individual's needs and circumstances.

Yes. Our services are regulated by the relevant care regulators, including the Care Quality Commission (CQC) in England and the Care Inspectorate Wales (CIW) in Wales.

This provides reassurance that care is delivered within robust clinical, governance and safeguarding frameworks, and is regularly assessed against recognised standards for quality, safety and effectiveness.

Active Assistance is registered for both Personal Care and the Treatment of Disease, Disorder or Injury, reflecting our ability to support individuals with complex health needs as well as day-to-day care requirements.

Funding arrangements vary depending on individual circumstances.

Support may be funded privately, through local authority funding, direct payments, NHS pathways or other arrangements. Our team can help explain the options available.
